We really value any feedback you have on your experiences of the Middle Years programme. This has been its first year, and we are keen to hear what you found helpful, as well as any suggestions for amendments and improvements.

Please download this short QUESTIONNAIRRE and return it to Dr Celia Woolf, Evaluation Coordinator for IPE, via e-mail (c.a.woolf@qmul.ac.uk).

In order to keep your responses anonymous, please save the questionnaire to your computer, fill it in, then return it to Dr Woolf as an attachment. The document will be kept but your e-mail will be deleted so there will be no record of who sent the response.

If you cannot complete the questionnaire electronically, please just print it off, fill it in and return to the postal address on the Contact Us page.
